Please click on the link below to download a copy of the CCHC Registration Form for Juniors for the 2012/13 hockey season.

Please print it out, complete it and bring it along with a cheque to your child’s first training session (for times of the training sessions, please see the training information on this site).

We need the Registration Form completed before your child can play – if you want to try a session first before committing to the season, please feel free to bring along a completed form in the first week and, should your child wish to continue playing, bring along the cheque the following week.
